Important Notice for Patients: This mailbox is monitored for customer service issues only. Please telephone your physician's office directly with questions or concerns regarding your care.

The physician you are scheduled to see during your initial consultation will, in most instances, become your primary provider of specialty services within our practice.

Other members of the NYOH staff who may be involved in your care – as determined by your diagnosis and the types of treatments prescribed – include nurse practitioners, physician assistants, oncology nurses, radiation therapists, laboratory technicians, pharmacists and CT and PET technologists.

Our multi-disciplinary team also includes schedulers, Patient Benefits Representatives, research coordinators and numerous other clinical and administrative staff who share a common dedication to excellence in patient care.

  • At the time of your initial and subsequent visits, please provide us with any changes to your contact information such as mailing address, insurance coverage and phone number(s).
  • Please remember that co-pays, as determined by your insurance carrier, are due at the time of your visit. For your convenience, we accept MasterCard, Visa and Discover Card.
  • Bring a list of questions that you may have and a pen and paper for jotting down notes or instructions. We encourage you to bring your NYOH folder and its contents to each appointment to help organize, maintain and update important information.
  • Whenever possible, consider having a friend or family member accompany you – an extra pair of ears can help ensure good communications.

We will do everything possible to provide you with a satisfactory experience. Patient satisfaction surveys are mailed randomly to patients who are seen at all NYOH locations. Participation is voluntary and anonymous. We encourage you to share your observations and suggestions for improvement.
