At New York Oncology Hematology, we believe that Advance Care Planning (ACP) is an important topic of discussion for all adults – regardless of age or health status. It's an opportunity to express one's preferences for life-sustaining treatment in the event an unforeseen illness or accident should occur and to name a health care agent (proxy) who is authorized to speak on a patient's behalf should he or she be unable to do so.
There are trained ACP facilitators at each New York Oncology Hematology site who can help foster dialogue between patients, family members and health care providers. Feel free to ask your NYOH physician or any member of our health care team for information about Advance Care Planning.

- Understanding the different types of advance care planning documents and forms
- How to make and communicate end-of-life decisions
- Learning to be comfortable about expressing personal values, beliefs and wishes
